At present there is very limited scientific evidence on the health effects of e-cigarettes.

Despite electronic cigarettes often being seen as a safer alternative to smoking, some evidence suggests they may affect lung function.

Preliminary results presented in September 2012 at the European Respiratory Society’s Annual Congress in Vienna found that 'smoking' or 'vaping' an e-cigarette for 10 minutes caused an increase in airway resistance, obstructing the air getting into and out of the lungs. In healthy subjects who had never smoked there was a statistically significant increase in airway resistance.

The study was based on a very small sample of people: eight had never smoked and 24 were smokers of whom 11 had normal lung function and 13 had either chronic obstructive pulmonary disease (COPD) or asthma.

The findings add to the growing interest in electronic cigarettes as a potentially healthier delivery system for nicotine and a more socially acceptable way of 'smoking'.

In August 2012, a study found that electronic cigarettes do not appear to be bad forheart function. Researchers compared the heart function of 20 daily smokers before and after smoking one tobacco cigarette to that of 22 e-cigarette users before and after using the device for seven minutes. The people studied were healthy and varied in age from 25 to 45.

The authors said that heart function got worse in the tobacco smokers and that their blood pressure and heart rate rose. People using e-cigarettes experienced only a slight elevation in blood pressure.

A 2010 briefing paper by Action on Smoking and Health (ASH) said one of the problems in assessing the safety of e-cigarettes stemmed from the number of models on the market that have not been tested.

In 2010, researchers from the Institute of Human Performance and Rehabilitation in Greece, wrote in the BMJ that "our knowledge on the acute and long term effects of e-cigarette use is, at best, very limited" because there are no human or animal studies of the effects of smoking e-cigarettes. They also pointed to a lack of any data on the effects of passively breathing in the nicotine vapour.

However, in considering whether e-cigarettes pose a threat to health, it should be remembered that smoking tobacco is the greatest single cause of avoidable ill-health and death, accounting for 80,000 deaths each year in England alone.

On this basis, ASH says that "e-cigarettes, which deliver nicotine without the harmful toxins found in tobacco smoke, are likely to be a safer alternative to smoking". However, it supports calls to regulate e-cigarettes as part of reform of products containing nicotine.
